【目的】
膝関節内反は、過去の研究から様々な傷害との関係が報告されている。しかしながら、下肢筋強化のトレーニングは、個人のアライメントを考慮して実施されていない。そのため、同様のトレーニングを行ってもアライメントの差異により、下肢の骨格、筋に及ぼす影響は異なる可能性がある。そこで本研究では、CKCトレーニング時の膝関節内反の有無による下肢への影響を筋硬度、筋電図、MRI、X線を用いて検討し、膝関節内反の有無による下肢のメカニズムを明らかにすることを目的とした。
【方法・結果】
研究課題1-1：膝関節内反の有無がレッグプレス後の下肢筋硬度変化に及ぼす影響
研究課題1-1では、正常膝群４名８脚、内反膝群６名１２脚を対象として、ナロウ、ミディアム、ワイド（両足関節外果間距離それぞれ30cm、60cm、90cm、下肢内外旋0°、15°、30°）の３種のスタンスを用いてレッグプレスを行わせ、各スタンスでオールアウトきたした直後と試技前の筋硬度を比較した。内反膝群のナロウスタンスにおいて、VL（外側広筋）とTA（前脛骨筋）が正常膝群よりも有意に高値を示した。しかし、スタンスが広がるに従い低値を示した。
研究課題1-2：膝関節内反の有無がレッグプレス中の下肢筋活動量に及ぼす影響
研究課題1-2では、研究課題1-1の内反膝群の被験者１名を除く同様の被験者を対象とした。各被験者には、75％/1RMで課題1-1同様の３種類のスタンスでレッグプレスを行わせ、その時の％iEMGを比較した。また、大腿四頭筋の伸展時におけるVM（内側広筋）とVLの活動を比較するために、VMのiEMGをVLのiEMGで除したVM/ＶＬ比を算出した。内反膝群におけるSOL（ヒラメ筋）の％iEMGは、大腿筋群と同レベルまで活動し、ナロウ、ミディアムスタンスにおいては正常膝群よりも有意に高値を示した。しかし、ワイドスタンスを用いることで有意に低い値を示した。VM/VL比は、ナロウ、ミディアム、ワイドスタンスそれぞれ、正常膝群0.98、0.98、1.02、内反膝群2.19、1.78、1.79であった。内反膝群のVMは、VLよりも活動していることが分かった。
研究課題2-1：膝関節内反の有無と下肢筋横断面積の関連性
被験者は、12名の健常男性（正常膝群6名、内反膝群6名）を対象とした。MRIは、日立メディコ社製AIRISmate0.2Tを用い、T1強調像を得た。測定は、各被験者の左脚とし、仰臥位にて大腿骨および脛骨全体をスキャンした。得られた画像から、下肢各筋の横断面積を分析画像全体の筋横断面積で除し、横断面積に対する各筋の構成比を検討した。VMの割合は、正常膝群で25.1%、内反膝群で30.7%であり、有意な差がみられた。その他の筋には、有意差はみられなかった。大腿部において内反膝群は、正常膝群よりもVMの占める割合が大きいことが分かった。
研究課題2-2：膝関節の内反の有無と荷重負荷がスタンスの変化に伴うFTAに与える影響
被験者は、12名の健常男性（正常膝群6名、内反膝群6名）を対象とした。各被験者の左脚の膝蓋骨がX線フレームの中央、管球面から100cmとなるように立たせた。そして、4種類のスタンス（50%/肩幅、100%/肩幅、150%/肩幅、200%/肩幅）を使用し立位を保持させ、荷重時は体重と同じ重量のバーベルを担がせた。50%/肩幅、50%/肩幅荷重、100%/肩幅、100%/肩幅荷重、150%/肩幅、150%/肩幅荷重、200%/肩幅、200%/肩幅荷重時の左膝関節前後像を撮影し、その時のFTAを比較した。膝関節内反の有無に関わらず、200%/肩幅時に、自重時から荷重時にFTAが増加し有意差がみられた。つまり、膝関節の内反の有無に関わらず、200
%/肩幅での荷重では内反傾向がみられ、内反膝群ではより内反を助長することになる。
【考察】
下肢へのCKCトレーニングを行う際、膝関節の内反の有無により明らかに筋・骨格へ及ぼす影響は異なることが、以上の研究より明らかになった。膝関節内反を呈するにも関わらず、正常膝群と同様の下肢トレーニングを実施することで、目的とする効果が得られないばかりか、筋への局所負荷や内反を助長する可能性も示唆された。150%/肩幅のスタンスは、下腿への負荷も軽減し、膝関節内反を助長する傾向もみられないため、推奨される方法ではないかと考えられる。 -
